I will not be participating in this offer because I don't buy these products. However, if I were to, this is what I would do:

1. e-mail the contact as specified in my post
2. I would begin the e-mail with something like: The information below is in response to your free rubber ducky offer. (You could also use Free Rubber Ducky Offer as the subject for your e-mail.) These are the items I purchased:
3. make a list of the products purchased including the UPC number, product name, and size.
4. Provide name & mailing address (don't forget that!)

I can't imagine what else they would need so you should be good to go.
