I got an email today which read:
Thank you for your recent participation in our "Food Lion Customer Panel – Q3 2009” survey. We greatly appreciate your time and effort in giving us your feedback.
As you may remember, once you completed the survey, your name was entered into a sweepstakes drawing. We are happy to inform you that you have been selected as the winner of a $25 Food Lion or Harvey’s gift card prize! Your choice! Please check either the Food Lion or the Harvey’s gift card box on the prize affidavit. Food Lion gift cards are redeemable at Food Lion, Bloom, Bottom Dollar and Reid's. Harvey’s gift cards are redeemable at Harvey’s only.
In order to receive your prize, you MUST complete and return the Prize Winner Affidavit form found at the following web site. Please print it and return this completed form to us by October 20, 2009.
