Hmmm, I do see where it says this, but if you look on the actual form, the fine print that is, it says 8-24-09-6-30-2010 but nothing about needing a purchase. Thanks for catching that!

You do need to include a proof of purchase with form

Has anyone done this? Is it just a rebate or is it some kind of coupon to give my dentist? I don't wanna mess with it and then find out my dentist won't even accept whatever it is they are sending.

You do need to include a proof of purchase with form
can you show me where it says you need to include UPC? I only see receipt with price circled.

I think by "proof of purchase" zydeco is referring to the receipt as it proves that you bought the product, not necessarily a UPC.
