Heineken GC Rebate - Where did you get your forms?

I am in NY, if I put a store that's not in NY, will the rebate be rejected?

Don't want to hijack the thread but does anyone know if we can use a PO Box for these? I've heard the forms don't say (don't have a form yet) but just curious if anyone has called yet or know for sure! Thanks!!

Hmm, I emailed them before I saw your answer and they said a PO Box is fine. Are you a beer purchase state?

I found mine in Shaws on Maine. Also I still have a few so I checked on the P.O. Box question. In very small print under the word address it says (No P.O. Boxes). I don't have a P.O. Box but I've seen it so many times that I would never use a P.O. Box for a rebate.

we don't ever get forms in our state and I am glad someone in this board traded them for just a stamp for each form. she even agreed paypal equivalent hf plus add'l .44 to cover postage to save me 1 stamp :)

when it first came out, those who had them were asking as much as $2 per form so I'm glad I waited.
