Need all members help please

We are working on a mini project to help our new members to HCW find the information they need here at HCW and we need to collect a little information from our members about what stores are in what states.

So if you could help us out by filling out this simple form so we know what stores are located in what states that will help us out immensly. There is also sections for other stores (that we don't have forums for) which will help us make decisions down the road about new forums to add.

Have you tried to refresh the page? I had that problem once, and refreshed, and it showed up.

Maybe choose hy-vee and then put the stores in the "I have these, that are not on the list' box?

Was forced to pick a drug store and we don;t have any yet. Walgreens has plans to open here in 2009, so I picked that one so I could submit. I did mark some stores that are in Anchorage, which is not my locale, but is "close". I think there are some that are there, but I don't know for sure.

I filed it out. Only FOUR stores are within 15 minutes of me; Walgreens. PicNSav, Piggly Wiggly and a standard Walmart. ALL other stores I marked are within an hour radius.

Some people drive :) and others don't.
