1. BUY any of the products listed on this form at participating Ace Hardware stores between 01/20/11 and 02/28/11.
2. SAVE your receipt(s) from your purchases.
3. When omitting credit card number from receipt, use pen or marker.
4. FULLY COMPLETE this rebate form including name and address.
5. FILL in the “TOTAL REBATE EARNED” box.
6. MAIL the following: this completed form, your original dated Ace Hardware cash register receipt and ORIGINAL UPC bar code(s) from each product package.

Note: Exceptions to including the original UPC are listed on this form. For these items only, you should hand-write the 12 digit UPC code on the form in the space provided.

° When signing in for the first time, you must use your 11 digit Ace Rewards number. It can be found:

• On the bottom of your receipt (at some store locations, only 10 digits will appear, the 11th digit may not be printed on the receipt. If this is the case, please refer to your Ace Rewards cardfor the last digit.)
• On the back of your Ace Rewards card.
• By calling our toll free number: 888-827-4223

° Only one rebate claim will be accepted during a promotion. Any other
claims must be submitted by mail using this form.

° For mail in and online submissions, please allow 8 weeks for processing. If you do not receive your rebate after 8 weeks, you may call 1-800-477-4137 or to track your rebate online at ACE Rebate Center.

• Limit of ONE rebate claim form per name, household or address. Rebate limits apply, see front for details.
• Register receipts must be dated between Jan. 20, 2011 and Feb. 28, 2011. Requests must be postmarked or submitted online by 03/31/11.
• Offers are product specific. Substitutions or rainchecks will not be honored.
• Include all rebates you wish to redeem within one submission. For mail-in rebates, mail all receipts from each shopping visit. For online, no receipts
are required. Receive ONE CHECK approximately 8 weeks after submission.
• Submissions from different addresses or different Ace Rewards numbers cannot be combined in one envelope or online submission.
• Not valid with any other offer.
• No photocopied, altered, substitute, counterfeit or non Ace receipts will be accepted. (In RI a photocopied or duplicate receipt is acceptable.)
