I did this back in March. I have an infant that is still doing Well-Baby check ups regularly - so I already had the credit card receipt from a Dr. visit.

I sent in the form from the Lysol website, the original Walgreens receipt, a copy of my card receipt from the Dr., and a copy of the detail from the Dr. office.

The detail from the Dr. office has a lot of information, but not mine. It is all of the procedures and the Dr. circled what was performed. At the bottom it has my name and my son's name, our address and our insurance info. But I had the Dr.'s office fax this to me, so it is blurry and they couldn't read the numbers if they had to. My advice would be to black out that info if you don't want them to see it. (your name and address are already on the Lysol form)

I received my $5 rebate very quickly!!

PS - I purchased Lysol wipes on sale B1G1 $2.79. Had 2 50cent Mfg Qs. And received the $5 rebate. - AND the packages were shrink-wrapped B1G1. (I had already taken my son to the Dr. so I didn't really count that in the cost). So 4 pks Lysol wipes for $3.21 Overage!! Great deal!! Especially for those with little ones that go to the Dr. all the time anyway......

I use these like crazy to clean all the baby toys and equipment!
