This is an old thread but thanks to the original poster as well as to the person who bumped it today!!! I wish I'd come across this 18 months ago! I'm bringing the forms to DHs doctor this afternoon and hopefully in 3 weeks he'll be set and we'll be saving $80 per month and moving from generic to actual drug.
